Michigan Community Health Network (MCHN)

The Michigan Community Health Network (MCHN) is a clinically integrated network formed by Michigan community health centers for the purpose of group contracting with Michigan Medicaid Health Plans. It was formed in 2015 and is owned by the Michigan Primary Care Association and 39 of its member health centers. MCHN negotiates value-based agreements on behalf of its members for the purpose of improving overall outcomes of the patients it serves and sharing in the resulting cost savings.

MCHN seeks to support health centers financial sustainability through increasing access to care, improving HEDIS measure scores and decreasing utilization and total cost of care. It supports these efforts through the creation of actionable data reporting, supporting technological solutions to improve outreach and engagement (for example, Automated Text Messaging, Direct Mail), and supporting best practice workflows and process improvement activities. Payor partnerships exist to support the work of RN Care Manager and Community Health Works to assess and eliminate barrier to care.

Agreements

MCHN has agreements with four Medicaid health plans and is participating in a Medicare REACH ACO. The agreements cover over 200,000 lives and the efforts of the health centers have resulting in lower costs each year from its inception proving additional funding to support the health center mission of serving the underserved as safety net providers of care.
